The Lakshmi Narasimha Swamy Temple stands at the foot of Mangalagiri Hill and is the larger and more visited of the town's two main shrines. The dominant feature is its eastern gopuram — an eleven-storey gateway tower built in 1809 by the local Raja Vasireddi Venkatadri Naidu — which is one of the tallest gopurams in coastal Andhra.

Inside, the shrine to Lakshmi Narasimha is carved in black stone, with a smaller shrine to Andal alongside. The temple complex covers several courtyards and the inner enclosures are still in active daily use; visitors are welcome to enter the main mandapa, but inner sanctum access varies depending on the festival schedule.
