Carunchio is accessible only by car. Public transport to this part of inland Abruzzo is essentially nonexistent. The village sits just off the SS650, which connects the Adriatic coast at Vasto to the interior; from the coast the drive takes about 30 minutes. From Lanciano, allow 45 minutes.
From Rome, the most practical route is the A25 autostrada toward Pescara, then south on the A14 and off toward Vasto. Allow around 3 hours.
