Miches has no direct transport from Santo Domingo or Santiago — you approach via connecting roads from Higüey, El Seibo, or Sabaná de la Mar. From Higüey, public minibuses (guaguas) run via Otra Banda and take about two hours. From El Seibo, the road south is more direct and takes around 90 minutes. From Sabaná de la Mar to the north, a short ferry crosses the bay to the road leading into Miches — the combined journey takes about an hour if the ferry is running.
The nearest airports are in Punta Cana (about 60 kilometres southwest by road) and Samaná's El Catey airport (roughly 50 kilometres northwest). Neither has regular public transport to Miches, so you'll need a taxi or a rental car from the airport.
